17-year old assistent farmer presents with a rapid grown of a 1 cm large flesh-coloured tumor at PIP dig II dext. Soreness, but no pain or associated complaints except for reduced mobility associated with the growth of the tumor.

What is the diagnosis and what treatment should be considered?

The diagnosis is Orf – a viral infection with parapox virus from lambs and goats – contracted while slaughting or nursing infected animals.
- After an incubation period of 5 or 6 days the tumor process presents with blood-tinged pustules and bullae.
- The lesions are often solitary. Secondary lymfangitis and reactive erythema multiforme may occur.
Treatment: Treatment is seldom necessary unless secondary bacterial infection occurs. Larger lesions may require a surgery excision.
